Outreach Trainers

The OSHA Training Institute education center that issued your current trainer card is your student card source.

In order to receive student course-completion wallet cards, you will be required to submit:

A fully-completed training program report, including the trainer information and certification section
A list of student names
A copy of your trainer course certificate of completion or trainer card, if you do not have an ID number.

Requests will be reviewed for valid trainer authorization, guideline compliance and completeness of information. Valid, correctly completed requests will be fulfilled within 3 weeks of receipt.
